All submissions must be accompanied by completed feedback forms from all team members. Please follow the steps below to guide you along.

  1. Click on the button below to be directed to the feedback form.
  2. Send the link to all your team members; all members are required to complete the form.
  3. Once you have completed the feedback form, a confirmation code will be shown at the bottom of the form.
  4. Copy the code and save it in a secure place.
  5. In the Registration form, enter the confirmation codes for each team member in their respective fields.
